CommonSpirit Health Community Health Improvement Grant

The CommonSpirit Health Community Health Improvement Grants support nonprofit-led projects that improve community health, advance health equity, and strengthen local systems of care in participating hospital service areas. Eligible projects must address priorities identified in a participating hospital’s Community Health Needs Assessment and serve vulnerable or underserved populations through sustained activities lasting nine to twelve months.
