The PacificSource Foundation for Health Improvement provides support to nonprofit organizations working to improve health status and meet healthcare needs in the communities the company serves in Idaho, Montana, and Oregon. The Foundation’s priority areas include the following: 1) access to healthcare services for children and youth, as well as adults with barriers to care; 2) healthcare innovations, with a focus on integrated healthcare, care for chronic disease, care coordination, healthcare through a patient-centered primary care home or medical home model, and prenatal and postnatal care for pregnant woman and infants; and 3) health and wellness, with a focus on encouraging physical activity, eating fruits and vegetables, tobacco prevention and cessation education, and parent education programs. The Foundation provides support through Healthcare Access Grants and Strategic Grants. Letters of intent for both grant programs may be submitted throughout the year. The PacificSource Foundation for Health Improvement provides resources and funds for the health and welfare of underserved and vulnerable populations, with an emphasis on children and youth.
